Phần 1: Cập nhật một số nghiên cứu về hiều quả/hiệu lực của thuốc Thiabendazole trong điều trị một số bệnh do ký sinh trùng ở người

            Trong xu hướng toàn cầu hóavà sự giao lưu, du lịch sinh thái rộng khắp của người dân đi từ các vùng không có bệnh lưu hành đến vùng có bệnh lưu hành và mang mầm bệnh quay trở ngược lại các quốc gia của họ làm lan rộng các mầm bệnh virus, vi khuẩn, ký sinh trùng,…Đặc biệt một số bệnh trong đó vốn dĩ gọi là căn bệnh nhiệt đới bị lãng quên trong nhóm các bệnh do ký sinh trùng lại khá phổ biến tại các quốc gia trong khu vực Tây Thái Bình Dương và châu Phi, nên gánh nặng bệnh tật hiện không phải là nhỏ mà cần phải quan tâm nếu chúng ta đều muốn loại trừ chúng như xu hướng hiện nay.

Hiện nay, một số bệnh do giun sán được Tổ chức Y tế thế giới (TCYTTG) xem như và xếp loại vào danh mục các bệnh nhiệt đới bị lãng quên (NTDs) ở nhiều quốc gia, trong đó có Việt Nam. Việc điều trị bệnh bằng thuốc đặc hiệu với hiệu lực cao có nhiều loại thuốc phổ dụng và chấp nhận dùng đường uống với hiệu quả cao và dung nạp tốt, ít tác dụng phụ như albendazole (ALB), mebendazole (MZ), pyrantel pamoate (PP), ivermectine (IVM), thiabendazole (TBZ), triclabendazole (TCBZ), praziquantel (PZQ). Trong đó, đáng chú ý loại thuốc thiabendazole là thuốc được sử dụng để điều trị nhiều loại giun khác nhau đã tỏ ra có hiệu quả tốt, an toàn và ít tác dụng ngoại ý.


Công thức thuốc thiabendazole


Thuốc Thiabendazole (biệt dược Vazigoc)

Trước khi đi vào các nghiên cứu đã thực hiện đa trung tâm trên toàn cầu với thuốc thiabendazole (TBZ) thì chúng ta cần nhìn nhận một vấn đề là bất cứ loại thuốc nào, kể cả vitamine đều có tác dụng ngoại ý hay tác dụng phụ cả, từ thấp đến cao, song nhiều tác giả chưa bao giờ dùng thuốc TBZ để điều trị mà chỉ nghe truyền miệng hay đọc vài ba thông tin trên tờ hướng dẫn và trên mạng lại quy kết quá lớn về tác dụng ngoại ý của thuốc TBZ có vẻ hơi phóng đại, điều này dẫn đến nhiều bệnh nhân mất cơ hội dùng thuốc TBZ này để điều trị bệnh cho họ. Trong thực hành lâm sàng cho thấy những tác dụng ngoại ý mà các thấy thuốc lâm sàng hay gặp khi điều trị với thuốc ALB, MB, PZQ, TCBZ,…đều có thể gặp khi dùng thuốc TBZ và hiếm thấy trường hợp nào phải gián đoạn dùng thuốc với các biệt dược hiện nay đang dùng như, hơn nữa khi đọc các thông tin về tác dụng phụ/ tác dụng ngoại ý và thận trọng khi dùng thuốc TBZ thì chúng ta nên đọc kỹ và có suy luận dưới góc độ y học với các dẫn liệu tiền lâm sàng trên mô hình thực nghiệm động vật thử nghiệm vẫn chưa có biểu hiện gì ngay cả khi các nhà thử nghiệm cho liều cao hơn liều dùng ở người gấp nhiều lần (để nắm thêm thông tin về thuốc TBZ, xin đọc các nghiên cứu đa trung tâm và cơ bản nhất về loại thuốc này tại trang website này là tốt nhất, bạn sẽ có đầy đủ các kết quả nghiên cứu https://www.sciencedirect.com/topics/chemistry/thiabendazole).


Thiabendazole có nhiều tên biệt dược khác nhau (Mintazole, Niczen, Vazigoc) được sản xuất dưới dạng viên nén, hàm lượng 500mg. Đây là một loại thuốc đặc hiệu có phổ rộng dùng để điều trị nhiều loại ký sinh trùng giun sán, đặc biệt là các loại giun tròn như giun đũa, ấu trùng giun lươn Strongyloides stercoralis, giun móc, mỏ, giun tóc Trichuris trichiura, giun móc chó/ mèo Ancylostoma braziliense, giun đũa chó Toxocara canis, giun đũa mèo Toxocara cati và giun kim Enterobius vermicularis. Tuy nhiên, hiện nay các nhà ký sinh trùng đã chứng minh thuốc còn có hiệu quả cao trong điều trị các hội chứng ấu trùng di chuyển nội tạng và di chuyển dưới da niêm mạc do Toxocara spp., giun lươn, giun đầu gai,…với liều thấp và liệu trình dùng ngắn ngày.

Về đặc tính dựơc lực học, thuốc TBZ có hiệu lực cao trên cả thể ấu trùng và con trưởng thành vì không những ngăn chặn giun đẻ trứng để phát triển thành ấu trùng mà còn ngăn chặn sự phát triển trứng hay ấu trùng thành con trưởng thành khi chúng bị đào thải theo phân ra môi trường.


Thiabendazole được sử dụng khá rộng rãi ở nhiều quốc gia trên thế giới, đặc biệt tại các nước châu Âu và Đông Nam Á-nơi có bệnh lưu hành. Thuốc chỉ định điều trị đối với bệnh do giun lươn, ấu trùng giun di chuyển dưới da, ấu trùng di chuyển phủ tạng như não, mắt, gan do nhiễm giun đũa chó/ mèo và các loại giun tròn khác có hiệu quả, nghĩa là thuốc TBZ có tác dụng trên đa giun sán chứ không phải một loại nào duy nhất, do vậy nếu có sự đồng nhiễm nhiều loại giun tròn nói trên thì vài trò của thuốc TBZ lại càng rõ ràng hay nói đúng hơn là mũi tên trúng đa đích. Đồng thời thuốc cũng được sử dụng để điều trị bệnh giun xoắn, nhiễm đa giun mà các thuốc điều trị khác không đạt hiệu quả hay chống chỉ định dùng. Tuy nhiên, do hiện nay các loại giun móc/ mỏ, giun dũa, giun tóc, giun kim vẫn còn nhạy cảm cao với thuốc albendazole và mebendazole nên thuốc TBZ chỉ dùng khi nào các thuốc trên không có hiệu quả hoặc kháng thuốc. Điều này, cho thấy một lần nữa thuốc có hiệu lực cao trong điều trị các loại giun sán.

Về tương tác thuốc, TBZ cạnh tranh với theophyllin ở vị trí chuyển hóa trong gan, vì vậy nó làm tăng nồng độ những hợp chất này trong huyết thanh lên đến ngưỡng liều độc.Tương tự, khi sử dụng đồng thời TBZ với những dẫn xuất của xanthine thì phải tiên liệu trước và phải theo dõi nồng độ thuốc trong máu, hay giảm liều.Việc sử dụng cùng lúc với các thuốc khác cũng phải được giám sát chặt chẽ. Hiếm khi TBZ làm tăng thoáng qua chỉ số xét nghiệm chức năng gan.

Liều lượng thuốc điều trị phải được bác sĩ chỉ định. Lưu ý người lớn không được uống quá 3 gam tương đương với 6 viên trong 1 ngày và thuốc chống chỉ định dùng cho người bệnh nhân có trọng lượng nhỏ hơn 13,60 kg. Thực tế tùy theo cân nặng của cơ thể để chỉ định liều dùng cho phù hợp như: nặng 13,60 kg uống 250mg tương ứng 1/2 viên một lần, nặng 22,60 kg uống 500mg tương ứng 1 viên một lần, nặng 34 kg uống 750mg tương ứng 1,5 viên một lần, nặng 45kg uống 1.000mg ương ứng 2 viên một lần, nặng 56kg uống 1.250mg tương ứng 2,5 viên một lần, nặng 68kg uống 1.500mg tương ứng 3 viên một lần. Thông thường uống 2 lần trong một ngày duy nhất. Thuốc được khuyến cáo uống sau bữa ăn để phòng tránh các tác dụng ngoài ý muốn (theo bảng khuyến cáo).

TBZ thuộc nhóm thuốc điều trị bệnh ký sinh trùng, chống nhiễm khuẩn, kháng virus, kháng nấm với dạng bào chế thành viên nén, hàm lượng 500mg, đóng hộp 4 vỉ x 7 viên.

Về cơ chế tác dụng của TBZ trên tất cả các loại ký sinh trùng chưa được biết một cách chính xác và thấu đáo, có thể là do TBZ ức chế enzyme fumarate reductase đặc hiệu của giun. Về mặt được động học, trên người thấy thuốc TBZ được hấp thu nhanh và đạt nồng độ đỉnh trong máu sau khi dùng thuốc 1-2 giờ. TBZ được chuyển hóa gần như hoàn toàn thành thể 5-hydroxy. Ở dạng này, nó hiện diện trong nước tiểu dưới dạng kết hợp với glucuronid  hay sulfat. Trong 48 giờ, khoảng 5% liều uống TBZ được tìm thấy trong phân và 90% trong nước tiểu. Hầu hết thuốc được đào thải trong 24 giờ đầu.

Thuốc TBZ dùng trong trường hợp khi các phương pháp điều trị khác không hiệu quả hay không thể dùng được hoặc phác đồ cần thêm thuốc thứ hai để trị các loại giun như giun móc, mỏ (Necator americanus/Ancylostoma duodenale), giun tóc, giun đũa.

Thuốc thường dùng sau bữa ăn 15-30 phút, nên nhai viên thuốc trước khi nuốt. Liều dùng TBZ theo sự chỉ dẫn của bác sĩ hoặc dược sĩ của bạn. Liều tối đa trong 1 ngày theo khuyến cáo là 6 viên Vazigoc (tương đương 3 gam Thiabendazole). Không được dùng TBZ cho bệnh nhân có trọng lượng dưới 13,6 kg, bệnh nhân dị ứng với các thành phần của thuốc và liều dùng thông thường là 2 lần mỗi ngày, tùy theo cân nặng bệnh nhân.

- Đối với giun lươn: dùng TBZ 2 lần/một ngày x 2 ngày liên tiếp;

- Đối với ấu trùng di chuyển dưới da (CLMs): dùng TBZ 2 lần/một ngày x 2 ngày liên tiếp Nếu ngưng thuốc 2 ngày mà triệu chứng, tổn thương vẫn còn, nên cân nhắc đợt điều trị thứ 2;

- Đối với ấu trùng giun di chuyển nội tạng: Dùng TBZ 2 lần/một ngày x 7 ngày liên tiếp.

Một số biểu hiện và cách xử trí khi dùng quá liều TBZ:

·Khi dùng quá liều TBZ sẽ biểu hiện rối loạn thị giác thoảng qua và những rối loạn tâm thần kinh, khi đó dùng phương pháp điều trị triệu chứng và hỗ trợ chức năng như làm cho nôn mửa hay súc rửa dạ dày để làm giảm lượng thuốc.

·TBZ chỉ dùng cho những bệnh nhân đã biết rõ nhiễm giun qua chẩn đoán xét nghiệm xác định, không được dùng như là thuốc phòng bệnh. TBZ không nên dùng ở phác đồ ưu tiên để điều trị giun kim Enterobius vermicularis, chỉ để dùng trong trường hợp những bệnh nhân dùng thuốc khác thất bại hay có phản ứng nhạy cảm/ dị ứng xảy ra thì phải ngưng sử dụng thuốc.

Một số tác dụng ngoại ý và thận trọng với thuốc TBZ

·Tiêu hóa: chán ăn, buồn nôn, nôn mửa, tiêu chảy, đau thượng vị, đau bụng, vàng da, ứ mật, tổn thương nhu mô gan và suy gan nếu dùng liều cao;

·Thần kinh trung ương: chóng mặt, mệt mỏi, buồn ngủ, choáng váng, đau đầu, tê liệt, tăng kích ứng, co giật, nhầm lẫn, trầm cảm, cảm giác bồng bềnh, yếu và thiếu phối hợp nếu dùng liều cao;

·Giác quan đặc biệt: ù tai, cảm giác bất thường trong mắt, nhìn mờ, giảm thị lực, khô màng nhầy (miệng, mắt, hội chứng sicca;

·Tim mạch: hạ huyết áp;

·Chuyển hóa: tăng đường huyết;

·Huyết học: giảm bạch cầu thoáng qua;

·Niệu - sinh dục: tiểu máu, đái dầm, nước tiểu có mùi khó chịu, tinh thể niệu;

·Quá mẫn: ngứa, sốt, rát mặt, ớn lạnh, phù mạch, phản vệ, ban da, ban đỏ đa dạng và hạch bạch huyết;

·TBZ không thích hợp cho việc điều trị nhiễm hỗn hợp nhiều loại giun đũa do có khả năng làm cho chúng di chuyển, gây biến chứng. Nên ưu tiên điều trị triệu chứng trước khi điều trị giun ở các bệnh nhân bị thiếu máu, mất nước hoặc suy dinh dưỡng. Những bệnh nhân có bệnh vàng da, giảm tiết mật, tổn thương nhu mô gan phải được thông báo với bác sĩ;

·Bệnh nhân rối loạn chức năng gan, thận cần được theo dõi chặt chẽ;

·Trong một vài trường hợp rất hiếm gặp, tổn thương gan có thể xấu đi và không có khả năng làm giảm đi tổn thương. Do tác dụng phụ trên TKTU có thể xảy ra, nên người bệnh cần phải tránh các hoạt động về thần kinh, đòi hỏi sự tỉnh táo;

·Khả năng gây ung thư, đột biến gen, giảm khả năng sinh sản? TBZ đã được nghiên cứu ngắn hạn cũng như dài hạn trên động vật ở liều lớn hơn 15 lần liều sử dụng bình thường ở người, thì không thấy tác động gây ung thư;

·TBZ không ảnh hưởng bất lợi về khả năng sinh sản trên chuột ở 2 1/2 lần liều sử dụng bình thường ở người hay trên chuột cống ở liều tương đương với liều sử dụng bình thường ở người;

·TBZ cũng không làm biến đổi gen ở thử nghiệm biến đổi gen vi khuẩn, thử nghiệm vi nhân (thử nghiệm micronucleus) trong in vivo và định lượng những chất trung gian chính trong in vivo;

·Trong Nhi khoa: Không sử dụng cho bệnh nhân nhi có trọng lượng dưới 13,6 kg.

·Bệnh nhân cao tuổi: Chưa có nghiên cứu đầy đủ về sự khác biệt giữa người cao tuổi (≥ 65 tuổi) với người trẻ trong việc dùng TBZ. Nói chung, cần thận trọng khi chỉ định liều cho bệnh nhân cao tuổi, nên bắt đầu từ liều thấp nhất, thường xuyên theo dõi sự suy giảm chức năng gan, thận, tim và giám sát những bệnh lý khác cũng như những loại thuốc khác mà họ đang điều trị;

·Thuốc TBZ được chuyển hóa hoàn toàn ở gan và chất chuyển hóa được bài tiết ra do thận. Vì vậy, nguy cơ độc hại cho thận sẽ lớn hơn nhiều ở những người suy thận. Do đa số bệnh nhân lớn tuổi đều suy giảm chức năng thận, nên khi sử dụng cho đối tượng này phải chọn lựa liều cẩn thận và phải theo dõi chức năng thận thường xuyên;

·Với các phụ nữ mang thai: Nghiên cứu về sự sinh sản và khả năng gây quái thai trên thỏ với liều gấp 15 lần liều bình thường sử dụng cho người, trên chuột cống với liều tương đương liều sử dụng cho người và trên chuột với liều gấp 2,5 lần liều bình thường sử dụng cho người, không thấy chứng cứ nào nguy hại đến bào thai. Trong một nghiên cứu bổ sung trên chuột, không thấy khiếm khuyết nào khi sử dụng TBZ ở dạng hỗn dịch trong nước với liều gấp 10 lần liều bình thường sử dụng cho người. Tuy nhiên, đã quan sát thấy khiếm khuyết hở hàm ếch và khiếm khuyết ở xương trục khi sử dụng TBZ ở dạng hỗn dịch trong dầu oliu với liều tương tự.Do chưa có nghiên cứu đầy đủ và có kiểm soát ở phụ nữ mang thai, chỉ sử dụng TBZ trong thai kỳ khi lợi ích vượt trội so với nguy cơ tiềm ẩn cho thai nhi;

·Với các phụ nữ đang cho con bú: Do không biết TBZ có bài tiết qua sữa mẹ hay không, nên không sử dụng cho người mẹ đang cho con bú;

·Vì thuốc TBZ khi dùng liều cao có thể gây chóng mặt và buồn ngủ, do đó không nên sử dụng cho người lái xe và vận hành máy móc.
