Phần 1. Các nghiên cứu sinh học phân tử trong phát hiện người lành mang ký sinh trùng không triệu chứng: giá trị cốt lõi trong loại trừ sốt rét
Trong 10 năm trở lại đây, số bệnh nhân sốt rét và số ký sinh trùng sốt rét có xu hướng giảm dần và ít đi ở nhiều vùng sốt rét lưu hành trên toàn cầu và ngay cả Việt Nam. Tuy nhiên, số ca mang ký sinh trùng không biểu hiện triệu chứng (asymptomatic carriers) hay số ca có ký sinh trùng thấp dưới ngưỡng phát hiện của kính hiển vi và test chẩn đoán nhanh (submicroscopic/ undermicroscopy detection)còn tồn tại nhiều trong cộng đồng là một mối đe dọa hay là một rào cản về mặt kỹ thuật trong lộ trình tiến tới loại trừ sốt rét trên thế giới cũng như tại Việt Nam.
Gần đây, nhiều nghiên cứu phát triển các kỹ thuật sinh học phân tử hiện đại có độ nhạy, độ đặc hiệu cvaf độ chính xác cao đã giúp phát hiện một tỷ lệ rất lớn trong cộng đồng về người lành mang trùng như thế bằng các ky thuật LAMP, nested-PCR, RT-PCR hay ultra-PCR. Điều đó đặt ra câu hỏi làm thế nào để định nghĩa mà xác định một ngưỡng tỷ lệ dương tính trong cộng đồng để làm thế nào chúng ta có thể can thiệp bằng các chiến lược khác nhau, chẳng hạn điều trị chỉ định thuốc hàng loạt (mass drug administration_MDA) trong cộng đồng hay không thì đến nay vẫn còn chưa có khuyến cáo chính thức từ các cơ quan chức năng. Do vậy, nghiên cứu tiếp cận các khía cạnh như thế là rất cần thiết
